Salary and grade: £29,439 - £32,909 per annum plus pay enhancements (Grade 7)

Hours per week: 37 hours per week worked on a rota basis of any 5 days out of 7 during hours 7 am to 11 pm, for which enhancements will be paid. A rolling rota exists to ensure service delivery.

Location of post: Service Office Base – The Quays, Baglan Energy Park. Will work across various locations within Neath Port Talbot.

Neath Port Talbot Community Wellbeing Team work closely with the Swansea Bay University Health Board to provide a Reablement service as well as short and long term care to individuals living within the Neath Port Talbot County Borough. We are actively seeking dedicated and trustworthy individuals who are passionate about the wellbeing of vulnerable people in Neath Port Talbot Borough.
To apply for this role you must be able to work as an effective team member of a multi-disciplinary, multi-agency team and take part in co-ordinating and liaising with professionals, individuals, and Community Wellbeing Team Support Workers to ensure a seamless transition between all service providers. You must also ensure the delivery of a high quality service in line with Registration and Inspections Social Care (Wales) Act 2016, statutory duties, policies, and procedures.
You must have experience of working with individuals in need of care or support in a health and social care setting; have an understanding of supporting people living with dementia; and have experience of developing quality assurance systems, and performance frameworks.
You must also have QCF qualification Level 3 or equivalent qualification in Care & be registered with Registration and Inspection of Social Care (Wales) Act 2016. QCF qualification Level 3 in Management would be an advantage.
It is essential that you can travel extensively within the NPT County Borough therefore a full valid UK driving licence and access to a vehicle insured for Class 1 business use is essential.
