We are currently recruiting an
Outreach
Worker to work as part of our busy multi-disciplinary team. Acting as the key liaison between Change Grow Live services, local hostels and shelters, the local council, and partners, this is a fantastic opportunity for anyone looking to actively engage individuals with the services they need to support improved health, wellbeing, social inclusion, and appropriate accommodation.

Responsibilities:
About the role:
Establishing effective communication and referral pathways those individuals who are homeless and in contact with other health and care agencies- Work closely with the local authority, Health & Social care sector, partners and any provision linked to Homelessness and improve joint working, attending multi-disciplinary meetings when as and when required
- Supporting service users from point of entry into the service and through their treatment/recovery journey
- Providing screening, assessment, and recovery planning and onward referral
- Coordinate the transfer of any accommodated out of area individual. This may include referral to and liaison with their new drug and alcohol service and pharmacy and provision of bridging prescription until an appointment and prescription is provided. Promoting carer, service user and community involvement
- Providing advocacy for access to partnership services
- Working with service users to support social (re)integration, enabling them to lead meaningful and purposeful lives: promoting recovery, resilience, peer support and self-determination
About you:
Prior knowledge and experience of working with people who are homeless, sleeping rough or sofa surfing- Have a strong knowledge and understanding of substance misuse issues
- Experience of engaging with a wide variety of individuals with a variety of backgrounds and needs
- Be an excellent communicator, both verbal and written with good IT skills
- Proactive and able to work on own initiative with excellent time management and prioritising skills
- Have a good working knowledge of mental health interventions, services, and good practice
- Be a supportive team player with strong interpersonal skills with the ability to work in partnership with a wide range of professionals, agencies, and internal/external stakeholders
- Ability to manage change successfully in a way that prioritises the needs of service users
- Understand the importance of information governance processes and commit to follow and apply all necessary safeguards
