Job Description
The Dementia Team support those affected by dementia in Shropshire and Telford from diagnosis, through the rest of their journey. Occupational Therapy is an integral part of each element of this service. We are well respected and valued by colleagues, but most importantly with the patients and families whom we support. You will be providing specialist assessments to support diagnosis, identify sensory issues; assess functioning; identify solutions to expressions of unmet need; work closely with family and carers. You will work creatively with an evidenced based practice.
- Provide specialist Occupational Therapy Assessments and interventions.
- Provide specialist consultative knowledge and support to the MDT.
- Work flexibly, possess excellent communication and interpersonal skills and effective clinical reasoning. Take responsibility for your own caseload of service users and occasional care coordination. There will be balance between generic and profession specific work, including a team duty role.
- Your role will involve working closely with other agencies, families, carers and local communities.
- You will be able to function autonomously in multidisciplinary teams, with supervision from the Clinical Lead and support from specialist Occupational Therapists as well as the wider team.

As a Registered Occupational Therapist you will be part of a team providing a specialist dementia service. The service will deliver a pathway of care in accordance with the latest evidence-base, which puts the service user’s needs at the centre of the service.
You will have a level of knowledge and expertise acquired through in-depth experience of mental health issues underpinned by extensive post registration experience to develop skills, knowledge and an excellent values base. There is an expectation that all work is underpinned by theoretical and technical knowledge, in addition to practical experience gained from a degree/diploma in Occupational Therapy and other relevant training. Clinical service delivery will incorporate a range of graded therapeutic interventions using occupational performance to address skill deficits enabling the client in areas of self-maintenance, productivity and leisure.
You need to be aRegistered Occupational Therapist with:

Working primarily between Monday and Friday, with the potential of some evening working to meet service user, carer and family needs. Car is required.
