Job overview

To work in partnership with patients and carers and members of the multi-disciplinary team to offer support and advice and promote informed choice; maximising independence and optimising quality of life.

To ensure provision of a nursing assessments and high quality, safe, evidence based care to patients and carers with long term conditions, with the aim of maintaining patients in their own homes, whilst admitted to the virtual care.

To manage a clinical caseload within the Digital Health team.

To ensure best practice guidelines and performance standards are met.

To be an autonomous practitioner responsible for the assessment and treatment of patients with complex health and social care needs within the team.

Monday to Friday 9am-5pm


Main duties of the job

A unique opportunity has arisen to join the Digital Health Service.

The service is at the forefront of managing patients physical health Status remotely using Digital technology. The service provides remote health monitoring to patients with a positive COVID diagnosis through the development of Virtual wards and COVID Oximetry@ Home service.

The service works closely with Royal Cornwall Hospital , University Hospital Plymouth, Community Matrons ,Specialist Nurses from Cardiac and Respiratory services, Practice Nurses and GPs with prevention and early detection of deterioration at the forefront of our intervention.

The team intervenes when appropriate if readings trigger parameters being breached - to maintain patients clinical safety in their own homes and therefore preventing avoidable hospital admissions or identifying the need for admission.

A key part of our intervention pathway is the provision of Health education coaching to enable patients to understand their unique biomarkers which will enable them to manage their health independently when they are discharged from our service .
