Job Summary:
- As a key member of our team, you will play a significant role in service and quality improvement through audits of relevant work processes, sharing of best practices, and staying up-to-date with the latest guidance on prescribing and medicines optimization.
- You will be supported in your clinical learning and professional development journey through regular access to a GP clinical supervisor and other clinical pharmacists.
- Additionally, you will have the opportunity to undertake the Centre of Postgraduate Pharmacy Education GP Pharmacist Training Pathway, a fully-funded 18-month program followed by a fully-funded accredited Independent Prescribing Program.
- Successful completion of the program will make you eligible for pay progression.

Job description and main responsibilities:
- Utilise your expert knowledge of medicines to clinically assess and treat patients with specific disease areas.
- Have the opportunity to contribute to the delivery of effective daily processes for the transfer of care between hospitals and GPs by providing effective signposting, recommending medication changes, and communicating accordingly.
- Support colleagues and patients with medication queries and monitoring while taking responsibility for the care management of patients with chronic diseases.
- Undertake clinical medication reviews to proactively manage people with complex polypharmacy, focusing especially on the elderly, people in care homes, those with multiple co-morbidities (in particular frailty, COPD and asthma), and people with learning disabilities or autism.
- Provide specialist expertise in the use of medicines while helping to address both the public health and social care needs of patients.
- Through the use of audit and action planning, you will help to tackle inequalities and provide leadership on person-centred medicines optimization, including ensuring prescribers in the practice conserve antibiotics in line with local antimicrobial stewardship guidance, and quality improvement.
- Contribute to the General Practice quality and outcomes framework (QOF) and enhanced services, using structured medication reviews to support patients to take their medications to get the best from them, reduce waste, and promote self-care.
- You will contribute pharmaceutical advice for the development and implementation of new services that have medicinal components (e.g. advice on treatment pathways and patient information leaflets).
- Ensure practices are compliant with CQC standards where medicines are involved working closely with general practice teams.
- Take a lead role in some aspects of supporting further integration of General Practice with other healthcare teams to help improve patient outcomes, ensure better access to healthcare, and help manage general practice workload.
- Specifically, contribute to the Quality Improvement Schemes (QIS) for practices (e.g. to reduce prescribing spend and unplanned admissions) and develop relationships by working closely with other pharmacy professionals across Primary Care Networks and the wider health and social care system.
- Take a central role in the clinical aspects of shared care protocols, clinical research with medicines, liaison with specialist pharmacists, liaison with community pharmacists, and anticoagulation.
- Undertake the Centre for Pharmacy Postgraduate Education (CPPE) Primary Care Pharmacy Education Pathway and provide Clinical Supervision for assigned Pharmacy Technicians.

Desirable:
- Minimum 2 years experience as a pharmacist
- Previous experience working in a GP surgery
- Experience in medication reviews relevant in General Practice
- Experience of common acute and long-term conditions
